Buttock implants are made from semi-solid medical grade silicone. They are FDA approved; and unlike breast implants, cannot leak or rupture once implanted into the body.
Several types of implants are available for buttock enhancement. You can choose from directional and non-directional implants:
- Directional (or anatomical) implants are shaped similar to a teardrop to provide more fullness along the mid-section of the buttocks.
- Non-Directional (or round) implants provide patients with uniform projection, creating fullness at both the top and mid-section of the buttocks. Many patients prefer non-directional implants because in the event that the implant rotates, the look will remain the same.
Patients can also opt for custom made semi-solid silicone implants, which are more similar to non-directional implants.
Implants can either be smooth or textured. Studies have shown no benefit of one over the other. Dr. Smiley typically performs buttock enhancement surgery using textured implants, but either can be used. Dr. Smiley will discuss which type of implant is best suited for your body type and aesthetic goals.
Incision Options for Buttock Enhancement (using implants)
Dr. Smiley can insert your implants through three different incision sites.
The preferred technique is through one incision along the center of the sacrum (the crease of your buttocks). Patients prefer this method because the scar is far less visible.
Other options include:
- Two incisions at the top of the buttocks (which can be covered by underwear or a swimsuit)
- Under the crease of the buttocks (which can be hidden by the natural fold where the upper thigh meets the buttock)

During Buttock Augmentation Surgery
Buttock augmentation is performed under general anesthesia, which will be administered by a board certified anesthesiologist in one of our fully accredited facilities. Incisions are made at the site agreed upon prior to surgery and the implant is strategically placed to create a natural looking, fuller backside.
Implants can be placed either above or below the muscle, or even under the fascia. Placement of the implant is determined by the patient’s body type and aesthetic goal. However, implants are typically placed under the muscle to provide the implant with more support, to prevent slippage and to protect against additional risks of infection. The ‘under the muscle’ technique, most often also provides patients with a more natural looking and feeling result.
Often time, patients opt for liposuction of the lower back to further enhance the appearance of the backside.
Buttock Implants Risks
Buttock Implants carry a higher risk of infection, as compared to a Brazilian Butt Lift or Fill; however, antibiotics are administered before, during and after the procedure to help prevent infection.
How Long is the Recovery Time
After your buttock enhancement surgery you will be required to wear a compression garment for 2-3 weeks. You will also be advised to sit on an inflatable “donut” for about the same amount of time. Since the implant is placed on the mid to upper portion of the buttocks, sitting down will not negatively impact the results of your surgery as long as you are not slouching and are sitting ‘properly’ – on the upper part of your legs.
Immediately following your procedure, your implants will be very high on your buttocks (in the same fashion that breast implants are placed high right after surgery). You may also notice that your bottom has a “boxy” appearance. This is normal. You will be able to see your final result after about 3 months.
Pain medication will be prescribed as needed during recovery to alleviate any pain or discomfort you may experience.
